To avoid cancellation and reschedule, first time visit patients MUST fill the new patient forms PRIOR to showing up at the office or MUST show up at the office 30 minutes PRIOR to appointment time in order to give sufficient time to fill the forms. Please also bring any recent lab results and x-rays, or have your doctor fax them to us at 443-292-4892.
